Thank you for your interest in referring to Door 43.

Before proceeding, please refer to our service criteria.

We are an independent health and wellbeing service for young people aged between 13-25* who are residing in Sheffield, attending education in Sheffield (school/college/university), or registered with a Sheffield GP Practice.

If you are a young person, you can refer yourself to all of our services by completing the referral form below.

Statutory and community services, schools, colleges, universities, parents and carers can also refer a young person to us.
